Spouse's Employer Coverage

If your spouse becomes newly eligible under an employer sponsored plan:

You can enroll yourself, your Spouse and your dependents in AAACarolinas Medical, Dental, and Vision plans

You may cancel or change you or your dependent children’s tier of coverage election in Medical, Dental, and Vision if you wish to enroll in your spouse’s plan.

You must make qualified status changes within 30 days of the termination date of your spouse’s employer coverage.

Supporting documentation must be uploaded to Ultipro.

 

To add or change dependents:

Log into Ultipro. Select Personal on the Home Page, then select Contacts and ADD. Input your dependent information and check the dependent and/or beneficiary box.

Once you have edited and saved your dependent information, you can then make changes to your insurance plans by selecting Change My Benefits on the Home Page.

To enroll or change your health and welfare benefit elections, log into Ultipro. After submitting the QLE request, please upload supporting documentation by selecting the Add Employee Document tab under the Things I Can Do section on the Life Event homepage.
